Following the example of Lufthansa, anonymous reporting systems for errors or near errors have also been put in use in the medical field. This information can often provide valuable indications regarding improvements and prevention of errors in the future.

Tracking down errors with CIRS

With the Critical Incident Reporting System (CIRS), Schön Kliniken have introduced an anonymous incident reporting system. Staff can use this system to report critical incidents or errors that they believe almost resulted in injuries. The anonymity ensures that employees will even report incidents in cases when they would otherwise have to fear personal consequences. In this manner, CIRS helps to identify weak points in workflows before larger problems arise. CIRS is therefore recommended by the German Coalition for Patient Safety (Aktionsbündnis für Patientensicherheit e. V.) for implementation in all hospitals.
